Jay V. Lewis, DDS
Dr. Lewis earned his Bachelor of Science Degree at the University of Illinois in Champaign and his Doctor of Dental Surgery degree at the University of Illinois in Chicago. He and his wife Kathy are natives of Peru, where they still live. They are the parents of three children and the grandparents of nine. He has practiced in Logansport for more than three decades.
Julie, RDH
Dental Hygenist
Julie and her husband Scott have been residents of Logansport for more than 40 years. Originally from southern Indiana, Julie received her hygiene degree from the University of Southern Indiana, Evansville. They are the parents of two daughters and the grandparents of two. Julie has been providing dental hygiene services for more than 40 years, 20 of them for Logansport Family Dentistry.

Dental Hygienist
Cindy is a native of Logansport and a graduate of Logansport High School. She completed the dental Hygiene Program at USB and earned a Bachelor’s degree from Indiana University. She has been employed at Logansport Family Dentistry since 1995. Cindy and her husband Chris are the parents of two children and reside in Logansport.
Rachel, RDH
Dental Hygienist
Rachel earned her Bachelor’s degree from Manchester College and then graduated from the IPFW School of Dental Hygiene. She and her husband Andy have lived in Logansport since 2010. They are the parents of four boys, all attending school in the LCSC. She enjoys vacationing in Michigan and attending the myriad of extra-curricular activities her children participate in.

Dental Assistant
Amber is a native of Logansport and graduated from Logansport high School and Ivy Tech’s dental assisting program in Lafayette. She and her husband Bob are parents of one daughter and a son-in-law. She has been employed by Logansport Family Dentistry since 1999. She enjoys spending time with friends and family.
Olivia, CDA
Dental Assistant
Olivia is a 2015 graduate of the Ivy Tech dental assisting program in Lafayette. She and her husband Justin are parents to five children in their blended family. Olivia has a passion for outdoor activities that she shares with her husband, including boating, fishing, and hunting.
Kayleigh
Hygiene Coordinator
Kayleigh is married to Dylan, and they reside in Lucerne. She has been with Logansport Family Dentistry since 2022. She and Dylan are the proud parents of two black labs and love nothing more than taking them for rides in their golf cart.
Allison
Business Assistant
Allison is a native of Logansport with nearly 30 years experience as a dental assistant. A graduate of the Ivy Tech Dental Assisting Program in Lafayette, Allison is known for her commitment to excellence and professionalism. Outside of work she enjoys spending time with family, friends and caring for her dogs.
